The following media is required to qualify for coverage:
1) photo of the frame number, 2) photo or video of the issue, 3) photo of the part serial number (only applicable for serialized electronic components).
For the quickest service, please add relevant media to the "Attachments" field at the end of the form. If you do not have media available at the time of submission, you will have the option to add it later.
This is the name that's printed on the bike frame. Please notice any suffixes, which may indicate generational differences.
This is the serial number of the bike. Usually located on the front of the bike above the fork OR near the bottom of the seat tube.
